Transgender Activists Circumvent State Bans on Minors’ Medical Transition
Network helps families access gender transition treatments despite state restrictions.

Transgender activists are increasingly moving medical transition for minors across state lines as 27 states have enacted laws restricting puberty blockers, cross-sex hormones, and surgeries for gender transition. In response, a nationwide network has emerged to help families continue medically transitioning their children in defiance of the law.
Among the most prominent organizations is the Campaign for Southern Equality (CSE), a North Carolina-based LGBT advocacy nonprofit. In 2023, CSE launched the Trans Youth Emergency Project (TYEP), which has since expanded to all 27 states with restrictions on medical gender transition for minors. TYEP works with over 130 local and regional partners and has distributed more than $1 million in travel grants to over 1,200 families.
Undercover Investigation Reveals Network’s Tactics
An undercover City Journal investigation found that many of CSE’s recommended medical providers, with the organization’s knowledge, discussed ways to circumvent state restrictions. Some providers described using diagnoses other than gender dysphoria to obtain prescriptions or insurance coverage, a practice that has drawn fraud investigations. Others discussed mailing medications directly to patients’ homes, even though some state laws prohibit supplying these drugs to minors for gender transition.
During an undercover investigation, the reporter posed as a mother in Nashville, Tennessee, where gender-transition medications are prohibited. The reporter was connected to a CSE care navigator, who assured that nonbinary children could still receive puberty blockers, emphasizing that doctors should defer to a child’s understanding of their own body.
CSE claims to monitor state laws daily and maintains a list of providers based on age, location, and family preference. The organization also offers travel grants to help families cover transportation costs for medical appointments. However, some providers discussed conducting virtual appointments with patients in states where gender-transition medications are restricted, without reliable procedures to verify their location.
